Good thing that these have been labelled as prescription drugs lest the public starts misusing and overdosing. They are to be used 12 hourly for 5 days for people with mild to moderate COVID-19 who have a high risk of progressing to severe disease. Molnupiravir is currently available in India and has been approved for the treatment of adult patients with Covid-19 and with oxygen saturation greater than 93% and who have a high risk of progression of the disease including hospitalization or death. As per a report on Jan 06, 2022, Pfizer may offer its Covid antiviral drug Paxlovid at lower prices for low and middle income countries (LMICs), including India, as an interim measure as the cheaper generic versions may not be available until later this year. * An additional point to note is that despite the US FDA's EUA approval - our ICMR and Indian Government have not included Molnupiravir in the latest guidelines for Covid treatment due to "safety concerns". ---- AVAILABILITY IN INDIA: * Molnupiravir is available in India as: - Molnutor by Torrent Pharma - Cipmolnu by Cipla - Molxvir by Sun Pharma - Molnulup by Lupin - Molnaflu by Aurobindo Pharma - Movfor by Hetero Healthcare - Molaz by Azista and so on. * Paxlovid (nirmatrelvir + ritonavir) will soon be available by same name by Pfizer itself. Since its approval by US FDA, the high income have bought most of the supply in advance. The generic form of the drug combination will most likely be available by the end of 2022 but the commercial version should be available sooner.

The Toughest Triage — Allocating Ventilators in a Pandemic The Covid-19 pandemic has led to severe shortages of many essential goods and services, from hand sanitizers and N-95 masks to ICU beds and ventilators. Although rationing is not unprecedented, never before has the American public been faced with the prospect of having to ration medical goods and services on this scale. Of all the medical care that will have to be rationed, the most problematic will be mechanical ventilation. Several countries, but not the United States, have already experienced a shortage of ventilators. Although shortages of other goods and services may lead to deaths, in most cases it will be the combined effects of a variety of shortages that will result in worse outcomes. Mechanical ventilation is different. When patients’ breathing deteriorates to the point that they need a ventilator, there is typically only a limited window during which they can be saved. And when the machine is withdrawn from patients who are fully ventilator-dependent, they will usually die within minutes. Anticipating the need to allocate ventilators to the patients who are most likely to benefit, clinicians should proactively engage in discussions with patients and families regarding do-not-intubate orders for high-risk subgroups of patients before their health deteriorates. Once patients have already been placed on mechanical ventilation, decisions to withdraw it are especially fraught. In the weeks ahead, physicians in the United States may be asked to make decisions that they have never before had to face, and for which many of them will not be prepared. Though some people may denounce triage committees as “death panels,” in fact they would be just the opposite — their goal would be to save the most lives possible in a time of unprecedented crisis. Is it Covid Rash or Drug rash. *Chief Complaints* A 56 yr old male with rashes over face since 1 day. Pt was taking cocktail for covid suspicion including coronil . he is neg for covid rt pcr. He was in home isolation for 14 days and completed 7 to 10 days treatment . He is known Diabetic on glimipiride and teneligliptin along with atorvastatin .
