A female 37 year old presented to ED with concern for panic attack, palpitations, severe dyspnea, and lightheadedness. She has history of anxiety disorder and is on medication for the same. Diagnose the ECG attached below.

VT, get a thyroid profile done. May be thyrotoxicosis.

Monomorphiic VT CHANGING into polymorphic VT.ventricular rate about 300/minute.

Ventricular tachicardia

Wide QRS irregular tachycardia ,rate > 250/ min--- AF WITH WPW SYNDROME.

Urgent cardioversion.
