37 year old female presented in er with severe dyspnea cough fever from 4 to 5 days and breathing difficulty worsened from one day . On general examination her bp was normal but respiratory rate was high Around 18 to 20 per minute. Her sugar was high and Dka was suspected and confirmed on oral examination candidiasis was there. Other systemic examination was unremarkable but respiratory was bad as left side there was no air entry and few crackles but on right side air entry was present with spasm. She was admitted in icu and managed with insulin infusion fluid therapy bronchodilators and broad spectrum antibiotics and antifungal and planned to start ATT once confirmed. Preliminary reports showed raised TLC was 31000 platelets normal hb around 10.6 other parameters normal . Renal and hepatic profile normal . Abg was showing type I failure with mild acidosis . I hav enclosed xray chest and cect thorax for corelation kindly give your opinion . Nw day 4 patient has improved a lot thank you
